In order to be cleared for each sport, you MUST complete the following 4 steps.

***All physical forms must be completed by a parent and doctor prior to the start date of your sport. The state of CA only accepts physicals that are signed, stamped and dated from a MD, DO, or PA.  NP and RN signed physicals are not currently allowed.***

2.  Digital registration must be completed via : www.athleticclearance.com

Registration for all seasons is currently open.  Parents and their athletes will need to visit the site, create an account, and register for each sport they plan to play.

Please do not register for “All Sports”.  Register for only the sports you intend to play.  You can always log back in and add sports at a later time if you are undecided.

3.  Preseason concussion baseline testing.

ALL athletes will need to complete and pass a concussion baseline test, on campus prior to tryouts on designated months. The test takes about 35 minutes to complete. More information regarding the testing schedule will become available soon.

4. Clearance Tag

To receive your clearance tag, bring all documents for review and signatures to either Mr. Javier Margarito,  Mr. Calvin Wong, or Mr. Nick Bonacorsi before the first day of tryouts.
