Athletic Update as of 7/1/2020
Thank you for taking the time to read through this update and for getting your athletic clearances started. At this time, we still have not received any definitive information regarding the start of the fall season. The Central Coast Section has told us that they will have more information about the fall season by July 20th. The CCS has also commented that they will do everything they can to ensure that we still have three seasons and they remain committed to serving our athletes. It is important that we all take the needed steps to obtain athletic clearance so that we can be ready when the time comes that we can resume after school athletics in our state and section. Below is information on the athletic clearance process for this year including some big changes to concussion baseline testing and getting cleared. Please email Athletic Director, Nick Bonacorsi @ email@example.com if you have any questions or need any help at this time.
2020-21 Athletic Start Dates:
Fall = August 7th
Winter = November 2th
Spring = February 1st
In order to be cleared for each sport, you MUST complete the following 4 steps.
1. Complete physical exam part 1 and 2:
Locate forms here:
Part 1: FUHSD Physical Exam Pt. 1
Part 2: FUHSD Physical Exam Pt. 2
***All physical forms must be completed by a parent/guardian and doctor prior to the start date of your sport. The state of CA only accepts physicals that are signed, stamped and dated from a MD, DO, PA or NP. Physicals signed by a RN, LVN, PT or DC are NOT allowed.***
Physical forms will need to be scanned and uploaded into your athleticclearance.com account.
Your physical form must be SIGNED, STAMPED, and DATED.
2. Digital registration must be completed via : www.athleticclearance.com
Registration for all seasons is currently open. Parents and their athletes will need to visit the site, create an account, and register for each sport they plan to play.
Please do not register for “All Sports”. Register for only the sports you intend to play. You can always log back in and add sports at a later time if you are undecided.
3. Preseason concussion baseline testing.
UPDATE FOR 2020-21 : FUHSD has temporarily allowed for concussion baseline testing to be completed at home, under the supervision of a parent/guardian.
Visit the link above in order to access information about the test and testing environment. All tests completed without first signing the digital consent at the link above will be marked “invalid” and deleted from the system.
Please take this test seriously as it will be used as your baseline to evaluate possible concussion symptoms in the event of an injury. If this test is not completed prior to the start of your season, you will be held out of all live contact until it is done and logged.
4. Getting Cleared
Once you have completed ALL 3 of the steps above, please email the Athletic Director, Nick Bonacorsi and the Athletic Trainer, Javier Margarito to have your clearance processed.
The system will not notify us directly, so please email us both once you have completed the above steps so we can go in and clear you or your athlete.
